POSITION:               Community Health Transportation Worker

DEPARTMENT:      Lower Sioux Community Health Department

REPORTS TO:         Lower Sioux Community Health Director

SALARY:                  $15 per hour

STATUS:                   Temporary casual, potential for permanent, primarily days (M-F) with some early am and evening hours

  • Must pass a health care background check allowing candidate to work in Minnesota
  • Must possess a valid Minnesota driver’s license and be insurable under the tribe’s auto insurance

POSITION SUMMARY:

The Community Transportation worker provides transportation to medical and dental appointments if a community vehicle is available and if all resources (family/friends) have been exhausted.  Works with Community Health Representative to coordinate transports.  Works as part of the health care team including community health and Lower Sioux Health Care Center.   

Essential Duties and Responsibilities:

  • Manages patient transport schedule; enforces transportation policy
  • Reports the health care needs of clients to community health nurse
  • Interfaces with other departments in an appropriate manner to facilitate client care
  • Returns patient calls in a timely manner
  • Communicates with care givers as needed
  • Maintains patient confidentiality, privacy and dignity at all times
  • Maintains cleanliness of vehicle and work environment
  • Complies with OSHA standards for safety and infection prevention
  • Supports mission and values of Lower Sioux Health Care Center and Lower Sioux Indian Community
  • Maintains a customer and patient focus
  • Represents Lower Sioux Health Care Center and the Lower Sioux Indian Community in a positive and professional manner at all times.
  • Communicates openly and honestly with providers, leadership and coworkers
  • Follows dress code
  • Acts in a respectful manner and maintain a professional demeanor and presentation at all times
  • Other duties as assigned by Supervisor

Education and Experience:

  • High school diploma or successful completion of a GED preferred

Skills & Qualifications:

  • Ability to manage time effectively
  • Ability to communicate effectively using cell phone; returning calls
  • Knowledge and experience working with Native American communities, culture and tribal programs
  • Ability to maintain confidentiality
  • Must have a current driver’s license, reliable personal transportation, proof of current insurance coverage, and insurability under the tribe’s auto insurance policy
